Mental Capacity Reports & COP3 Assessment

Our reports and COP3 services are designed to support clients, families, and legal professionals in navigating complex capacity-related decisions with confidence and care.
We adopt a holistic and functional approach, considering not just the clinical presentation but also the psychosocial and contextual factors influencing decision-making. Our goal is to empower clients, providing them with the right information, support, and opportunities to maximise their ability to make informed choices.
Our assessments are collaborative, involving families, carers, and relevant professionals where appropriate, and we are experienced in supporting best interests decisions and helping resolve potential conflict. We place great emphasis on ethical practice and remain mindful of key legislation, including the Mental Capacity Act, Deprivation of Liberty Safeguards, and the forthcoming Liberty Protection Safeguards.
We are skilled in working with clients with complex needs, including those who are difficult to engage, present with fluctuating capacity, or exhibit the “frontal lobe paradox.”
For urgent matters, we can provide rapid-response capacity assessments (subject to availability and an additional fee).

Challenging Behaviour
Challenging behaviour can occur in the context of learning disability, neurodiversity, dementia, or following neurological illness or injury. It often presents as a constellation of behaviours that can significantly impact quality of life, wellbeing, and rehabilitation outcomes. Left unaddressed, challenging behaviour can disrupt engagement with professionals, hinder recovery, and place considerable emotional strain on loved ones and care teams.

We are experienced in using recognised tools and frameworks, including:
- ABC charts
- OAS-MNR
- SASBA
- SASNOS
- ABS
We are also working towards digital behaviour recording to improve data accuracy and streamline collaboration with multidisciplinary teams.
At Zenith Neuropsychology, we take a comprehensive, compassionate, and evidence-based approach to understanding and managing challenging behaviour. We aim to reduce distress, enhance engagement, and promote meaningful recovery or improved wellbeing for each client.
We use a holistic, biopsychosocial formulation to understand the origins of challenging behaviour. This involves:
- Direct observation of the individual
- Behavioural data collection and analysis
- Informant interviews and reports
- Functional analysis, which is central to identifying why behaviours are occurring
This structured understanding allows us to design positive behavioural support plans tailored to each client’s unique needs.
Risk assessment is a key part of our process, ensuring that safety is prioritised for clients, families, and care teams. We are experienced in working within relevant legal frameworks and can support services in aligning with appropriate legislation.
We draw upon principles of new learning methods and personalised intervention strategies to address behaviours such as aggression, disinhibition, and inappropriate sexual behaviour.
Psychological Therapy
At Zenith Neuropsychology, we provide one-to-one, evidence-based psychological therapies for clients seeking support with their mental health and emotional wellbeing.

Our mission is to help clients move towards a life that feels more meaningful and manageable, equipping them with the tools to navigate challenges, improve emotional resilience, and enhance overall quality of life.
We draw upon a range of therapeutic approaches, including Acceptance and Commitment Therapy (ACT), Compassion-Focused Therapy (CFT), Cognitive Behavioural Therapy (CBT), Cognitive Analytic Therapy (CAT)-informed approaches, and Eye Movement Desensitisation and Reprocessing (EMDR). This flexibility allows us to tailor interventions to each client’s unique needs, preferences, and goals.
We work with a wide range of presentations, including depression, anxiety, panic disorder, post-traumatic stress disorder (PTSD), health anxiety, psychological adjustment to neurological illness or injury, and difficulties related to other health conditions.
We also support clients with relational and interpersonal challenges, personality disorders, obsessive–compulsive disorder (OCD), neurodiversity-related concerns, anger management, and other complex psychiatric conditions.
Cognitive Assessment
We provide comprehensive cognitive assessments for adults designed to evaluate all key domains of thinking, memory, and behaviour.

Our assessments offer a detailed understanding of cognitive strengths and weaknesses to support diagnosis, rehabilitation planning, and personalised strategies that enhance daily functioning and independence.
We utilise the most current, evidence-based tools, combining digital assessments with traditional pen-and-paper tests, all of which represent the gold standard in clinical neuropsychology. Each assessment is culturally sensitive and customised to the individual’s background, ensuring fairness and clinical precision. Our approach integrates findings within a clear, functional framework, connecting test results to real-world implications for work, education, and daily life.
Zenith Neuropsychology provides expert, person-centred evaluations that meet the highest professional standards.
We accept referrals from private clients, case managers, solicitors, and other professionals. While we offer comprehensive clinical assessments, we are currently unable to accept medicolegal instructions for expert witness reports. Our assessments are available for clinical, rehabilitation, and personal understanding purposes only, and are not to be used for legal proceedings or court reports.

Assessment of Prolonged Disorders of Consciousness (PDOC)
At Zenith Neuropsychology, we provide specialist assessment for individuals with Prolonged Disorders of Consciousness (PDOC), following the most recent Royal College of Physicians (RCP) guidelines.

Our assessments use gold-standard observational tools, including the Wessex Head Injury Matrix (WHIM) and the Coma Recovery Scale–Revised (CRS-R), to support accurate diagnosis and guide care planning.
We take a highly scientific and robust approach, employing counterbalancing, repeat assessments, and standardised instructions to ensure reliable responses. Assessments are person-centred, incorporating detailed input from families, care teams, and other healthcare professionals to comprehensively understand each individual’s presentation. In line with RCP recommendations, we also use structured observational methods to assess low mood and pain, particularly in individuals who may have limited or inconsistent behavioural responses.
Our team offers support, psychoeducation, and practical advice to families and professionals, helping to direct management strategies and enhance understanding of awareness and recovery. We are experienced in providing recommendations to optimise daily routines, including suitable rest periods and meaningful sensory or cognitive activities, to maximise engagement and quality of care.
For individuals whose first language is not English, we have experience conducting assessments in collaboration with professional translators to ensure cultural and linguistic fairness.
We accept referrals from all sources, including private clients, case managers, and healthcare professionals.
Group Psychological Debriefs and Reflective Practice
At Zenith Neuropsychology, we provide group psychological debriefs for teams who have experienced major, traumatic, or distressing events in their workplace.

We can provide training to organisations wanting to develop their staff support and wellbeing packages.
Using a structured Post-Event Team Reflection model, sometimes referred to as a “cold debrief”, we offer teams a safe, facilitated space to reflect on challenging experiences once the immediate crisis has passed. These sessions are grounded in the principles of Psychological First Aid, aiming to normalise and validate emotional reactions, enhance team communication, and strengthen cohesion by creating a shared narrative of the event.
In addition, we offer training in the Team Immediate Meet (TIM) Tool, which equips care teams to conduct “hot debriefs” in the immediate aftermath of a difficult incident, fostering resilience and early emotional processing. For staff members who continue to struggle following traumatic or distressing experiences, we can provide individual staff support and follow-up sessions.
We also facilitate reflective practice groups for healthcare and care teams, offering a structured environment to consider the emotional impact of their work, explore team dynamics, and develop sustainable approaches to self-care and wellbeing within the healthcare context.
Learning Disability Assessments
We offer comprehensive assessments for adults with learning disabilities, using gold-standard cognitive assessments alongside detailed evaluations of adaptive skills across the lifespan.

Each assessment is conducted with sensitivity and a person-centred approach, ensuring that findings are meaningful, practical, and directly applicable to real-world settings.
Our assessments identify individual strengths and areas of difficulty, enabling us to develop tailored treatment plans, training goals, and support strategies that promote independence and enhance daily functioning.
We provide clear, evidence-based recommendations to monitor progress over time and guide ongoing care and intervention planning. Our assessments can also help to determine eligibility for services, including disability benefits, educational or workplace support, and community resources, as well as evaluate an individual’s capacity to live or work independently.
We provide clear, practical recommendations for employers, care teams, and families to help translate assessment findings into everyday support. Our guidance focuses on optimising communication, managing cognitive or emotional challenges, and fostering independence and wellbeing.
